Description

The Health Promoter will play a key role in developing, implementing, and evaluating health promotion initiatives aimed at improving the overall health and well-being of the community.

This position involves working closely with community members, stakeholders, and other healthcare professionals to identify health needs and create effective strategies for health promotion and awareness and community development.

Responsibilities

• Program Development: Design, implement, and evaluate health promotion programs tailored to the needs of the community.
• Community Engagement and Development: Build and maintain relationships with community members, local organizations, and stakeholders to promote health initiatives.
• Education and Outreach: Conduct workshops, seminars, and presentations on various health topics, such as nutrition, physical activity, mental health, and chronic disease prevention.
• Strategy Development: Develop strategic approaches to enhance health and ensure that local, regional, and national policies impacting public health are implemented in a way that promotes well-being.
• Partnership development: Develop partnerships with key individuals, communities, and organizations that can impact or influence public health, empowering these collaborations to more effectively promote health.
• Resource Development: Create and distribute educational materials, including brochures, flyers, and digital content, to raise  awareness about health issues and available services.
• Data Collection and Analysis: Gather and analyze data to assess the effectiveness of health promotion programs and identify areas for improvement.
• Advocacy: Advocate for policies and practices that promote health equity and improve access to healthcare services.
• Collaboration: Work with interdisciplinary teams within the health centre to integrate health promotion into all aspects of patient care as well as assess health inequities and work with stakeholders to propose solutions to reduce them.
• Communication and outreach: Promote CHC activities on social media platforms and the organization’s website to increase awareness and engagement.
• Community Leadership: Facilitate and lead various committees and supervise health promotion projects and initiatives.
• Project Planning: Manage projects and develop workplans that include goals, objectives and outcomes and ensure project deliverables are completed within the timeline and budget.
• Perform all other tasks as assigned by the Executive Director.

Qualifications

• Bachelor’s Degree obtained from a recognized university, in Health Promotion or a combination of undergraduate studies in a related field and in-depth experience.
• Excellent communication skills, group facilitation and interpersonal skills.
• Ensure the quality of services and programs and the smooth operation of the Center, in collaboration with all staff.
• Experience in developing, implementing, and evaluating health programs in different community settings.
• Knowledge of community resources, especially health and social services.
• Ability to work within an interdisciplinary team.
• Excellent oral and written communication skills in French and English.
• Proficiency in computer use (Microsoft Office, Internet, and email).
• Valid Ontario driver’s license and access to a car.
• A criminal records check will be required before hiring.

Work Conditions

• Full-time permanent
• 35 hours per week
• 4-day work week

• Salary range: Per the Ministry of Health salary grid
