Primary Care

Primary care services are delivered by an interdisciplinary clinical team consisting of nurse practitioners, physicians, registered nurses, social workers and a dietitian.

Each client is registered to the clinic and assigned a nurse practitioner as their primary care provider. Clients will see their primary care provider for the majority of their health care needs.

Our clinical team works together to coordinate the best care possible for your health needs. We encourage clients to take charge of their health and actively participate in their care.
