Description

We are seeking a compassionate and dedicated Registered Nurse to join our team. In this role, you will be responsible for managing and coordinating virtual healthcare services to ensure patients receive timely, efficient, and effective care. You will work closely with patients, families, and healthcare providers to optimize the delivery of virtual health services.

Responsibilities

• Coordinate and manage virtual care services for patients, including telehealth appointments, remote monitoring, and patient education.
• Serve as the primary point of contact for patients accessing virtual care, providing guidance and support throughout their care journey.
• Ensure the appropriate and timely coordination of consultations, and manage all information pertaining to virtual care services;
• Assist patients during their virtual visits by conducting exams using tele-diagnostic equipment;
• Conduct all physical exams required by the specialists prior to consultations;
• Identify partners and increase access to a wide range of services for the entire Sudbury East community;
• Support the primary health care team in order to meet the primary care needs of patients, their families and the community;
• Provide a wide range of primary care services to patients of the Centre de santé Univi Health Centre (CSUHC); conduct patient exams, follow-ups, triage, screening and advice.
• Ensure compliance with all relevant policies, procedures, and regulations related to virtual care.
• Collaborate with multidisciplinary teams to enhance the quality and continuity of patient care.
• Document patient interactions and care plans accurately and in a timely manner.
• Participate in the development and implementation of virtual care protocols and best practices.
• Adhere to policies and procedures as well as professional standards.
• Other duties as assigned by the Program Director.

Qualifications

• A Bachelor of Nursing or a Diploma in Nursing with relevant experience may be considered.
• Member in good standing with the College of Nurses of Ontario.
• Minimum of three (3) years experience in virtual care coordination, community or hospital setting, preferred.
• Knowledge in identifying and analyzing the needs of various age groups.
• Experience in planning, implementing and evaluating a variety of health-related topics in a community setting.
• Knowledge of community resources, including health and social services.
• Ability to work within an interdisciplinary team.
• Fluency in oral and written English and French.
• Valid driver’s license and access to a vehicle.
