Community Health and Wellbeing Month

October is Community Health and Wellbeing Month!

This year’s theme is Furthering Health Equity, Powering Connections.

At Univi, we’re Powering Connections to Health by helping our communities better understand and navigate primary health care.

Throughout October, we’ll be sharing practical information about accessing care, communicating with your healthcare team, planning ahead and making the most of the care available in our communities.

Strong primary health care is an important part of building healthier communities. Follow along throughout October as we share ways we can work together to strengthen connections to health.

Why do we ask why you’re calling?

When you tell us the reason for your visit, it helps our team understand what you need and determine the most appropriate next step.

The information you share can help us determine:
• The right provider or service
• The appropriate type of appointment
• How soon you may need to be seen

You don’t need to know who you should see or what type of appointment to book. That’s where our team can help.

The more we understand, the better we can help connect you to the right care.

Respect builds stronger connections.

Positive healthcare experiences are built on patience, listening, understanding and respectful communication.

Taking the time to listen, communicate clearly and ask questions when something isn’t clear can help build trust between patients and their healthcare team.

We all have a role in creating a healthcare environment where people feel safe, heard and respected.

Together, we can build stronger connections to health.

Plan ahead for prescription renewals, forms and paperwork.

A little planning can help give your healthcare team the time they need to safely review and process your request.

Prescription renewals: Request your renewal before your medication runs out. Renewals aren’t always automatic, and your healthcare provider may need to review your medication and health information or determine if follow-up is needed.

Forms and paperwork: Have a form, letter or other document that needs to be completed? Whenever possible, send it to Univi well before your deadline. Your healthcare provider may need time to review your information, gather additional details, speak with you or schedule an appointment.

Planning ahead can help prevent avoidable delays and interruptions in your care.

Your call can help someone else access care.

When you can’t attend an appointment, letting us know as soon as possible gives our team the opportunity to offer that time to another patient who may be waiting for care.

We know that plans change. By calling to cancel or reschedule, you can help us make better use of the appointments available in our community.

A simple call can make a difference for someone else.
