System Navigators

Navigating the health and social support systems can be confusing, intimidating and discouraging. If you are unsure of who to talk to about your specific needs, the System Navigators are the people at Univi that can help.

System Navigators collaborate closely with patients to identify their needs, provide comprehensive information about services, and assist in overcoming barriers to access. They work with the entire interdisciplinary team at Univi to make sure you get the care and support you need.

Services Include
  • Connecting you with local social services, whether it be to help with employment, education, food security, transportation, home care, housing, or parenting needs.
  • Assisting you with completing forms for Ontario Works, Canada Pension Plans, subsidized housing applications, ODSP Self-Report Package, etc.
  • Supporting you in advocating for services.
